Of all the skin cancers, malignant melanoma is the most serious. It accounts for approximately 75% of all skin cancer deaths and is the sixth most common form of cancer. This can be directly attributed to the fact that melanoma, unless treated early, usually metastasises to other organs.

The first place melanoma traditionally metastasises to are the lymph nodes near the site of the skin cancer. The lymph nodes are part of the lymphatic system and play an important role in immune system function. Special white blood cells called lymphocytes circulate in the body seeking out foreign cells. They return to the nodes where the foreign bodies are identified and the cells are specialised to fight that particular infection. From the node, they flow back into the blood stream, following it to the foreign cells, which they then attack.

When an infection is present, lymph nodes are commonly swollen. This is due to the large amounts of B cells, the specialised lymphocytes responsible for creating antibodies. When cancer is present, the nodes usually become enlarged and may appear as a hard knot. However, if there are only a few cancer cells, this is not always the case. If melanoma metastasises to the lymph nodes found in the chest or abdomen, patients may experience shortness of breath or backache.

Malignant melanoma may also spread to the liver, brain, lungs, or digestive tract. Cases of advanced ocular melanoma, or melanoma of the eye, metastasise to the liver in 95% of cases. In some cases, there are no symptoms of hepatic melanoma. Those that do experience symptoms may have nausea, weight loss, fever, jaundice, sweats or pain in the upper right area of the abdomen.

Approximately half of malignant melanoma tumors metastasise to the brain. Seizures are the most common symptom, appearing in 25 to 37% of patients. Symptoms can vary depending on where the tumors appear in the brain. However, in cases of metastatic brain cancer, some are more common. These include headaches, bleeding, nausea, vomiting, swelling, and impairment of motor or mental function. If sensory organs in the brain are involved, patients may have problems with hearing or vision as well.

Melanoma of the lung is rare. According to statistics, it is responsible for less than one percent of tumors in the lungs. There aren?t always symptoms of this condition. Some people do experience a cough that does not respond to usual treatment, shortness of breath or fluid buildup that leads to pleural effusion (collection of fluid next to the lung). If pleural effusion is present, patients may have discomfort in the chest.

In the digestive tract, melanoma is often misdiagnosed due to its tendency to mimic the appearance of other more common disorders. Symptoms are not always present, but may include difficulty swallowing, a full sensation in the abdomen, blockage of the digestive tract or pain in the abdominal area.

With malignant melanoma, preventing metastases is critical. Once tumors metastasise, they can have devastating effects on the body. In some cases, death may result.

Everyone should know the ABCD?s of melanoma; A for Asymmetry, B for Border, C for Colour and D for Diameter. If melanoma is suspected, it should be checked immediately. Early detection increases the chances of survival from what can otherwise a fatal disease.

Durable goods orders dip, but business spending up (Reuters)

WASHINGTON (Reuters) ? New orders for long-lasting manufactured goods slipped in August on weak demand for motor vehicles, but a rebound in a gauge of business spending supported views the economy would likely avoid another recession.

The Commerce Department said on Wednesday durable goods orders dipped 0.1 percent after a 4.1 percent jump in July.

Economists had forecast durable goods orders, items ranging from toasters to aircraft meant to last three years or more, would be unchanged last month.

Non-defense capital goods orders excluding aircraft, a closely watched proxy for business spending, increased 1.1 percent last month after a 0.2 percent fall in July.

This suggested that businesses, sitting on about $2 trillion in cash, had not responded to the recent financial market volatility by curtailing spending on capital goods.

Economists had expected a 0.3 percent rise in this category.

"It shows that we're not falling off a cliff, which helps. This is a sigh of relief, and slightly reassuring in the short-term," said Wayne Kaufman, chief market analyst at John Thomas Financial in New York.

U.S. stock index futures added to gains. U.S. bond prices on the long end pared gains, while the Euro trimmed gains versus the dollar.

Although manufacturing, which has done the heavy lifting for the fragile economy recovery, is slowing, August's durable goods report pointed to underlying resilience and offered hope output would continue to expand.

Orders last month were held back by an 8.5 percent drop in bookings for motor vehicles -- the largest decline since February last year.

The drop in orders came despite a 23.5 percent rise in orders for civilian aircraft last month.

Boeing received 127 orders for aircraft, according to the plane maker's website, up from 115 in July. Delta Airlines placed an order for 100 aircraft.

Excluding transportation, orders also slipped 0.1 percent after rising 0.7 percent in July. Economists had expected this category would also be unchanged.

Outside of transportation and primary metals, which fell 0.8 percent, details of the durable goods report were relatively strong.

Orders for machinery edged up 0.1 percent, while computers and electronic products rose 1.3 percent. Demand for capital goods increased 4.2 percent and electrical equipment and appliances rose 1.3 percent.

Shipments of non-defense capital goods orders excluding aircraft, which go into the calculation of gross domestic product, increased 2.8 percent after rising 0.4 percent in July.

TUCSON, Ariz. (AP) ? Prosecutors have again urged a judge to throw out a request by the Tucson shooting rampage suspect's lawyers to halt his forced medication.

Jared Lee Loughner's lawyers argue that U.S. District Judge Larry Burns should stop the forced medication of their mentally ill client with psychotropic drugs because it's unlawful.

A prison facility in Missouri where Loughner was being treated decided earlier this month that his forced medication needed to continue because he posed a danger to himself.

Prosecutors say the decision to medicate Loughner was lawful and that he was given due-process rights.

Loughner pleaded not guilty to 49 charges stemming from the Jan. 8 shooting that killed six and injured 13, including Rep. Gabrielle Giffords.

Loughner returned to Arizona on Monday after spending four months at the prison facility in Springfield, Mo., where experts were trying to make him mentally fit to stand trial.

Loughner faces a hearing Wednesday in Tucson where a judge will consider whether the 23-year-old can be made mentally fit for trial and whether to extend his four-month stay in Springfield, Mo., by another eight months.

Burns required Loughner's presence at the hearing in Tucson even though defense lawyers objected to it and argued that traveling would be disruptive for their mentally ill client.

Loughner wanted to attend the hearing because he wants to see his parents, who live in Tucson.

Four months ago, Burns found Loughner mentally unfit to stand trial after mental health experts concluded Loughner suffers from schizophrenia.

Prosecutors asked the judge to extend Loughner's stay at the Missouri prison by another eight months.

Loughner's attorneys argue that prosecutors haven't proven that there's a substantial probability their client's mental condition can be improved so that he can go to trial.

The struggles of a gay military family | Photographers

The United States became the 23rd of 26 NATO countries to allow military service by openly gay people last week. An estimated 66,000 lesbians, gay men and bisexuals are serving in the U.S. military, according to a recent study by UCLA?s Williams Institute. Many are still afraid to come out. I visited a gay military family to hear the story they are now able to tell.

A week ago, Luz Bautista, 30, and her fianc?e Alejandra Schwartz, 24, both Navy petty officers, were celebrating the end of the U.S. ban on openly gay service members.

This week, they?re being forced to live apart.

Bautista headed to Illinois Monday, away from Schwartz and their daughter Destiny, 6, for a three year posting that could be extended.

?The emotional toll. You can?t even describe it. It has been tearing us apart for the last couple of months,? said Bautista.

While the repeal of the 18-year-old ?Don?t Ask, Don?t Tell? policy marked a major advancement for gay rights, it doesn?t address many of the practical effects it has for gay troops, and exposes the challenges remaining for the military to accommodate couples.

Bautista, a petty officer 1st Class and Schwartz, petty officer 2nd class, who has served as an Arabic translator in Iraq, are good examples of the inequities that exist for gay service members.

Same-sex couples do not have the same spousal benefits or protection from being stationed separately that heterosexual married couples in the military have.

Coming out to her colleagues ? and on national television ? last week was one of Bautista?s proudest moments. While it ended an era of having to live in secrecy, it does little to change the situation she?s in with a committed partner who is also committed to a Navy career.

The timing of the change almost seems cruel in light of how difficult her life is getting. In addition to having to leave Schwartz, she?s pregnant with a second child.

?It?s going to be difficult for me,? Bautista said. ?I?m going to be by myself for the first time in 12 years? I?m scared.?

Even if the couple could travel to one of the few U.S. states where gay marriage is legal, the marriage would not be acknowledged by the military or by the state of California. Under the federal Defense of Marriage Act, the term ?spouse? only refers to opposite-sex married couples.

?As a result, a service member in a same-sex relationship with another service member is not eligible for co-location consideration,? according to Pentagon spokeswoman Cynthia Smith. ?Gay, lesbian or bisexual service members in a committed same-sex relationship like their unmarried heterosexual counterparts can make individual hardship-based requests for accommodation in assignment.?

Bautista did make a hardship claim, but at the time she couldn?t reveal that she had a partner. Instead, she relied on the other struggles she faced: as a pregnant, single mom, caring for her own disabled mother while raising a child and paying a mortgage and bills on a house she owns in San Diego.

It was not hardship enough.

?Unfortunately I got orders right before the repeal happened,? Bautista explained. ?So I couldn?t say, I?m gay and I have a fianc?e, can you guys help me out please? But I think that even if I had said it, it still wouldn?t have mattered because the rules are the rules. This marriage is not real to the Navy.?

Her family tells her she should leave the Navy. But she and both children would lose their health insurance, unable to be considered as Schwartz?s family dependents.

Six-year-old Destiny knows her mother is about to leave, and follows her everywhere. She has an infectious giggle and a wobbly front tooth. She reads in English and Spanish, and speaks a few words of Arabic that Schwartz has taught her.

?What am I going to do if I get out? I love the military. I want to keep serving. I want to help out this country. I want to still go on deployments, and I want to go back on a ship,? Bautista said.

If some changes don?t happen in the next three years, she and Schwartz could even be deployed on a ship at the same time ? away from their children.

?Her shore duty ends in 2013, and she goes to a ship,? Bautista said. ?Because it?s a rotation, you go from shore to sea. I don?t get transferred till 2014 to a ship, so by that time, both of us are going to be on a ship, and we?re going to have two kids.?
